Primary Care Opportunity making over $300,000 in Milwaukee, WI

COMPENSATION: $260,000 BASE, TOTAL POTENTIAL $312,000 + Full Benefits

We're a new, innovative healthcare provider devoted to improving the lives of our patients. We deliver best-in-class care at comfortable, accessible neighborhood clinics where our patients can feel at home and become part of a vibrant, wellness-focused community. Our patients experience greater continuity of care and the comfort of knowing they will be treated with respect by people who genuinely care about them, their families, and their communities. The center provides Uber/Lyft to those who don't drive. Also, they offer exercise, art, and other activities for their seniors to promote better health at no cost to the patient.

Role:

1. Compensation: Base $260,000 + up to 20% of base salary in bonus
2. Bonus based on: 1. Patient satisfaction rating 2. Living the company's values 3. HEDIS compliance 4. Reducing hospital sick days
3. Primary Care - Outpatient
4. Start date: New center opens May 2025
5. Value-Based Care Model
6. Patient population: Seniors with 3-5 complex diseases (heart disease, diabetes, hypertension, etc.)
7. Average patients per day: 10-12
8. Panel size: Max of 650
9. Shifts: Monday-Friday 8am-5pm
10. Call Schedule: Based on number of providers in the market (e.g., if 32 physicians in market, call 1:32 weeks)
11. Physicians have their own care team with MA/scheduler/referral specialist and full support from leadership

Requirements:

1. MD or DO
2. Board Certified/Board Eligible in Family Medicine or Internal Medicine (required)
3. Fellowship in Geriatrics is a plus but not required
4. Current, unrestricted, active license to practice medicine
5. Current DEA registration
6. Proficient PC skills in office procedures: suturing, joint injections, skin biopsies, basic wound care
