Job Description: Primary Care Physician (PCP)

The Primary Care Physician (PCP) works as a lead in our team-based care environment. We are a value-based care provider focused on quality of care for the patients we serve. The Physician's responsibilities include outpatient medicine, continuity of care, health maintenance, and disease prevention. They keep detailed medical histories and records, refer patients to specialists as needed, and begin to influence departmental strategy. The Physician makes decisions on moderately complex to complex issues regarding technical approaches for project components, often performing work independently and exercising considerable latitude in determining objectives and approaches.
