Lake County Health Department and Community Health Center seeks an Advanced Practice Registered Nurse (APRN) to provide direct clinical care, order labs, develop treatment plans, and prescribe medications within a collaborative agreement.

This full-time role requires IL Full Practice Authority, NP licensure, and national certification; duties include team collaboration, patient education, quality improvement, and ongoing training.

Qualifications

• Direct clinical care to clients, including assessment and planning.
• Supports Chronic Care Model to reduce health disparities.
• Provides clinical consultation and supervision as assigned.
• Adheres to National Patient Safety Goals and related plans.
• Participates in quality improvement activities and staff development.

Responsabilités

• Works with staff as a team to provide high quality care.
• Identifies and assesses health concerns and creates individualized plans.
• Develops and maintains Chronic Care Model processes.
• Provides clinical consultation and supervision to staff and students.
• Participates in mandatory trainings, meetings, and safety drills.
