Job summary

Hope For Youth is seeking a full-time Youth Peer Advocate (YPA) or Family Peer Advocate (FPA) to join the ACT program on Long Island, NY. The role focuses on delivering direct family therapy, transportation, and collaborative care within an interdisciplinary team.

Qualified candidates will have a Family Peer Advocate Credential or willingness to obtain it, lived experience as a caregiver, and proficiency with Microsoft Office and EHRs.

Qualifications

  • Lived experience as a parent or caregiver to a child with mental health diagnosis.
  • Ability to use Microsoft Office Suite and navigate EHR systems.
  • High school diploma or GED required; credential as Family Peer Advocate preferred or obtainable.

Responsibilities

  • Provide rehabilitative support services to families to benefit the child/youth.
  • Assist families with developing community connections and natural supports.
  • Deliver direct family therapeutic contact and transportation as needed.
  • Participate in daily team and family meetings; attend trainings and supervision.

Job description

Hope For Youth's (HFY) Youth Assertive Community Treatment (ACT) is a program licensed by the NY State Office of Mental Health and designed to address the significant needs of children ages 10 up to 21, who are at risk of entering, or are returning home from high intensity services, such as inpatient settings or residential services, using a multi-disciplinary team. Children with significant psychiatric needs, who are at risk of institutional level of care, require intensive interventions to adequately support the child and family's complex needs, to avoid high end services or facilitate and support a successful transition back to community.

Our team has an opening for a full-time Youth Peer Advocate (YPA) or a Family Peer Advocate (FPA) in the ACT program.

To be a successful Family Peer Advocate you must be ready, willing, and able to do the following always:
  • Function within the policies and procedures as outlined in program practice guide and employee manual
  • Adhere to state regulations and applicable code of conduct
  • Work within an interdisciplinary team to assist client in their overall functioning and achieve desired outcomes
  • Provide rehabilitative support services through an array of formal and non-formal services to the parent, family member or caregiver for the benefit of the child/youth.
  • Assist families with developing community connections and naturals supports to enhance the quality of life by integration and supports for families in their own communities, as well as helping the family to rediscover and reconnect to natural supports already present in their lives
  • Helping the family learn and practice strategies to support their child's positive behavior, assisting the family to implement strategies by clinicians, providing individual or group parent skill development related to the behavioral health needs of the child (i.e., training on special needs parenting skills).
  • Attend trainings and professional development as assigned
  • Attend daily team meetings and family meetings as assigned
  • Apply agency-identified Evidence Based Interventions best suited to meet the client's needs
  • Provide transportation for client to their service location as necessary
  • Perform other resulting and necessary work to meet the needs and requirements of the clients, the program, Hope For Youth, oversight agency and any other regulatory obligations
How you will generally spend your time in this job:
  • 60% Direct family therapeutic contact including transportation
  • 20% Case documentation, reports, case related meetings
  • 20% Staff/team meetings/supervision/training
Minimum Qualifications to have and maintain for this role are:
  • Family Peer Advocate Credential or willingness to obtain the credential within a set timeframe
  • Lived experience as a parent or caregiver to a child with mental health diagnosis
  • Computer proficiency, including Microsoft Office Suite, and experience working with EHRs
  • Highschool Diploma or GED
  • Valid NYS driver's license in good standing
  • Physical Requirements: read/write reports and log entries, walking/standing/sitting for prolonged periods, bending/stooping, kneeling/squatting, climbing stairs, reaching overhead, repetitive wrist/finger movements, daily driving own vehicle (mileage reimbursement available).

This Opportunity is being offered as Full Time, 35 hour work week (flexible hours)
