Qualifications

  • Experience with goal setting, case management and health-related services preferred.
  • Knowledge of culturally diverse populations and their needs.
  • Ability to obtain CPR/First Aid/AED certification and Food Handler’s Card.
  • Maintain confidentiality per program policy.

Responsibilities

  • Coordinate family support services and develop Family Partnership Agreements.
  • Maintain accurate records in family files and tracking databases.
  • Promote wellness through screenings, referrals, nutrition workshops and training.
  • Coordinate home visits and parent-teacher conferences with staff.
  • Recruit and train parents as volunteers and monitor hours.
  • Act as a liaison with community agencies for expectant families and children.
