You will provide case management services to youth in residence and their families, coordinating with social services to support treatment goals and family-centered practices. Key duties include assessing family resources, linking the youth and family to community supports, ensuring medical appointments are up to date, communicating with families and collaterals, and

Qualifications

• Bachelor's degree in Human Services or related field.
• Two years of experience working with children and families.
• Valid driver’s license and reliable transportation. Auto Insurance.
