**Job Description**

• Deliver patient care services through a remote technology platform.
• Provide holistic, evidence-based care inclusive of assessment, diagnosis, treatment, management of health problems, health counseling, and disposition planning for patients aged 18 months and above.
• Document encounters utilizing an electronic health record (EHR).
• Collaborate with a dedicated team of professionals to ensure the highest standard of care.
• Evaluate primary care, acute, chronically ill, and transitional care patients.
• Provide healthcare education and counseling, including STI prevention and contraceptive care.
• Maintain patient confidentiality in accordance with PHI and HIPAA standards.
