Pay And Benefits

Hourly Pay Range: $55.853432 - $71.063777

Salary offers will be made within the posted pay range and will be based on a candidate's experience (paid or unpaid) that is directly relevant to the position.

Generous Paid Time Off Package, Including

  • 12 hours of vacation accrued per month
    • Eligible newly hired employees will have the one-time option to frontload their first year of vacation accruals in compliance with EPP 66 - Vacation Leave ! This means you have access to vacation time at time of hire.
  • 8 hours of sick accrued per month
  • 10 paid holidays and 1 personal day per year
  • Monthly accruals (prorated for less than 1.0 FTE)

Employees must work a minimum of 30 hours per week to receive full benefits. Employees working 20 - 29 hours per week are eligible for medical benefits for themselves and family members. JOB DETAILS AND QUALIFICATIONS

We are looking for a Nurse Practitioner passionate about healthcare who is committed to working in a collaborative, team-based environment to join Clackamas County's Health clinic teams at our Sandy Health Clinic!

Clackamas Health Centers (CHC) provides fully integrated care to its patients in a dynamic, team-based environment. Our team of primary care providers are supported by a strong team of RNs, pharmacists (dispensing and clinical), behavioral health providers (including a strong primary care based MAT team), and dentists. We are looking for a family nurse practitioner passionate about caring for the underserved at our Sandy clinic. This position is a growth role- in other words, you get to build your own panel! This position will care for all ages, with more of a focus on adults. A strong interest in providing care for opioid use disorder is a plus.

Required Minimum Qualifications/ Transferrable Skills:*

  • Licensed as a Registered Nurse by the State of Oregon
  • Licensed and Board Certified as a Nurse Practitioner with prescriptive privileges (within 30 days of hire) by the State of Oregon
  • DEA registration certificate with Schedule 2 - Schedule 5 privileges (within 30 days of hire)
  • Must possess and maintain a Basic Life Support (BLS) certificate
  • Experience as a Nurse Practitioner in a family practice setting, with a focus on providing comprehensive primary care services

Preferred Special Qualifications/ Transferrable Skills:*

  • Experience working in an FQHC setting

TYPICAL TASKS

  • Provide comprehensive, patient-centered primary care across the lifespan within an integrated healthcare system.
  • Develop and implement personalized, evidence-based treatment plans to address acute and chronic conditions.
  • Deliver preventive care services, including screenings, immunizations, and health education to promote early detection and disease prevention.
  • Perform physical exams, including pediatric and women’s health evaluations, assessing growth, development, and overall well-being.
  • Diagnose and treat common medical and behavioral health conditions; prescribe and manage medications according to clinical guidelines.
  • Order, interpret, and analyze diagnostic and laboratory tests to inform clinical decisions and adjust treatment plans accordingly.
  • Educate patients and families on diagnoses, treatment options, medications, and health maintenance strategies.
  • Coordinate referrals to specialists, psychiatric services, and community-based social services to support comprehensive care.
  • Participate in clinic operations, quality improvement initiatives, and continuous process enhancements to improve care delivery and outcomes.
  • Collaborate on the development, review, and implementation of clinical policies, procedures, and practice standards.
  • Engage in professional development, including continuing education, training, and participation in clinical policy development.

WORK SCHEDULE

This position works 24 hours during a standard workweek of Monday through Friday (3 days/week; 8 hours/day). Specific hours of work will be discussed with the candidate selected for this position at the time an offer of employment is extended. This position is designated as on-site/in-person. It is expected that the selected candidate will perform duties at the specified on-site work location.
