Job summary

Squirrel Hill Health Center in Pittsburgh, PA seeks a Family Nurse Practitioner (FNP-C or FNP-BC) or Physician Assistant to deliver independent clinical care under the Chief Medical Officer. You will manage office, mobile unit, and home visits across ages, with prenatal care support in coordination with OB/GYN and prenatal teams.

You will join a mission-driven community health setting, providing comprehensive care and working with a diverse patient population.

Qualifications

• Advanced degree and certification in NP or PA with PA/NP licensure in PA.
• Active DEA registration or eligibility.
• Experience in community health settings preferred.

Responsibilities

• Conduct patient assessments, physical exams, diagnose acute and chronic conditions, and develop treatment plans.
• Orders or executes tests, analyses, and imaging to inform patient care.
• Analyzes test results and examination findings to diagnose conditions.
• Manage chronic diseases common in geriatric populations (diabetes, hypertension, dementia, polypharmacy, falls risk).
• Inoculates and vaccinates patients to immunize from diseases.
• Advises patients on diet, hygiene, and prevention strategies.
