Job summary

Children Express Care Clinic seeks a Family Nurse Practitioner to provide direct patient care in a part-time on-site role in the Greater Indianapolis area. You will conduct comprehensive assessments, diagnose and manage acute and chronic conditions, and develop individualized treatment plans for patients across the lifespan.

Responsibilities include primary and urgent care services, prescribing medications, ordering and interpreting diagnostic tests, and performing follow-up visits.

Qualifications

• Must hold RN license and FNP certification in Indiana.
• Master’s or Doctoral degree in Nursing.
• Experience in primary care, family practice, or home health is beneficial.
• Strong documentation skills and familiarity with electronic health records (EHR).
• Ability to work collaboratively with interdisciplinary teams and adapt to varying clinical environments.

Role Description

The Family Nurse Practitioner will provide direct patient care in a part-time, on-site role located in the Greater Indianapolis area. Day-to-day responsibilities include conducting comprehensive assessments, diagnosing and managing acute and chronic conditions, and developing individualized treatment plans for patients across the lifespan. The role includes providing primary care services, addressing urgent care needs within scope, prescribing medications, ordering and interpreting diagnostic tests, and performing follow-up visits. Additional responsibilities include documenting care in accordance with regulatory and organizational standards, educating patients and families on health management, and participating in quality improvement initiatives.

Qualifications

• Candidates should possess strong Nurse Practitioner skills, including independent clinical decision-making and effective patient communication.
• Candidates should possess solid Nursing skills with a focus on patient-centered, holistic care across diverse populations.
• Candidates should possess Primary Care skills, including preventative care, chronic disease management, and routine health screenings.
• Candidates should possess Medicine-related skills, such as clinical assessment, diagnosis, prescribing, and interpreting diagnostic results.
• Candidates should possess Urgent Care skills, including triage, management of non-emergent acute conditions, and timely intervention.
• Current Registered Nurse (RN) license and Family Nurse Practitioner (FNP) certification, with active and unrestricted license in the state of Indiana.
• Master’s or Doctoral degree in Nursing from an accredited program.
• Experience in primary care, family practice, or home health settings is highly beneficial.
