Job summary

SGS Consulting in Austin, TX seeks a Family Nurse Practitioner to provide comprehensive primary care, acute and chronic disease management, and patient education to patients aged 18 months and older. The role emphasizes counseling, care coordination within a patient-centered medical home, and adherence to HIPAA/PHI standards.

You will document all care in the EHR, collaborate with pharmacists and clinic staff, and contribute to revenue-cycle management.

Qualifications

• DEA number application is required at the time of offer acceptance.
• Bilingual (Spanish/English) candidates are highly preferred for this location.

Responsibilities

• Evaluate primary care, acute, chronically ill, and transitional care patients; provide healthcare education and counseling; disposition planning for patients aged 18 months and above.
• Provide patient counseling including pregnancy prevention, STI prevention/safer sex practices, contraceptive care counseling and medication management.
• Educate patients on health maintenance and respond to patient care inquiries.
• Document all patient care within an EHR according to Clinic policies and procedures.
• Coordinate care with internal and external colleagues within a patient-centered medical home to ensure high-quality care.
• Deliver excellent customer service and seek to increase patient engagement and satisfaction through feedback integration.
• Create a warm and welcoming environment by focusing on the patient and tailoring communication accordingly.
• Collaborate with pharmacy and front store colleagues to provide a complete patient experience.
• Maintain patient confidentiality per PHI and HIPAA standards.
• Participate in professional development and maintain professionalism in all interactions.
• Assist with hiring, development, and evaluation of practice employees.
• Complete clinic operations tasks including inventory, lab results follow-up, and billing processes.
• Own the success of the practice by implementing the clinic-level business plan.
