The Family Health Center – Southwest Indiana is seeking a Family Nurse Practitioner to deliver comprehensive primary care across the lifespan, including pediatric to geriatric patients. The NP will assess, diagnose, treat, educate, and coordinate care under applicable laws and collaborative practice agreements.

Qualifications

  • Master’s or Doctoral degree in Nursing
  • National certification as a Nurse Practitioner
  • Current prescriptive authority and DEA registration
  • Minimum two years of family practice or primary care experience preferred
  • Experience with pediatric through geriatric populations

Responsibilities

  • Provides primary care across the lifespan (pediatric to geriatric)

  • Diagnoses and manages acute and chronic conditions

Job description

The Family Nurse Practitioner provides comprehensive primary care services to patients across the lifespan, including pediatric, adolescent, adult, and geriatric populations. The practitioner is responsible for health assessment, diagnosis, treatment, health promotion, disease prevention, chronic disease management, patient and family education, and coordination of care across the continuum. Care is delivered within the practitioner’s education, certification, licensure, privilege, and scope of practice, and in accordance with applicable laws, regulations, organizational policies, and any required collaborative practice agreement.

The Family Nurse Practitioner works collaboratively within an interdisciplinary care team to improve access, quality outcomes, continuity of care, and patient satisfaction.

Essential Functions

The essential functions include, but are not limited to, the following:

  • -Provides primary care services across the lifespan, including pediatric, adolescent, adult, and geriatric care
  • -Performs age-appropriate health assessments, physical exams, and diagnostic evaluations
  • -Diagnoses and manages acute and chronic conditions across all age groups
  • -Orders and interprets laboratory tests, screenings, and diagnostic studies
  • -Prescribes medications and provides age-appropriate patient and family education
  • -Performs medication reconciliation and monitors treatment effectiveness
  • -Provides preventative care, wellness visits, immunization support, and health screenings
  • -Delivers anticipatory guidance based on developmental stage and patient needs
  • -Engages parents/guardians in care as appropriate and compliant with regulations
  • -Coordinates care with internal and external providers, schools, and community partners as appropriate
  • -Identifies behavioral health, social, and environmental factors impacting health
  • -Completes accurate, timely documentation in the electronic health record
  • -Supports appropriate coding, billing, and compliance requirements
  • -Monitors and follows up on test results and clinical communications
  • -Participates in quality improvement and patient safety initiatives
  • -Provides consultation and support to clinical staff as appropriate
  • -Maintains required licensure, certifications, and continuing education
  • -Adheres to all HIPAA, infection control, and regulatory standards
  • -Promotes teamwork, professionalism, and service excellence
  • -Performs other duties as assigned
Work-Related Experience
  • -Minimum two (2) years of experience in family practice or primary care preferred
  • -Experience working with pediatric through geriatric populations
  • -Experience with preventative care, immunizations, and chronic disease management
  • -Familiarity with family-centered care and care coordination preferred

  • -Dexterity to perform procedures (e.g., injections, point of care testing)
  • -Exposure to bodily fluids, infectious diseases, and hazardous materials
  • -Prolonged sitting at a desk with frequent computer use.
Working Conditions:
  • -Clinical environment with exposure to infectious diseases, hazardous materials, and emotionally challenging patient cases.
Education, Certification, and Licensure

Education: Master’s or Doctoral degree in Nursing

Certification: National certification as a Nurse Practitioner

Licensure: Current prescriptive authority and DEA registration
