Overview

Valley-Wide Health Systems, Inc. is looking to fill a Family Nurse Practitioner/Physician Assistant position to provide medical care at Guadalupe Health Center in Antonito, CO. We are a non-profit, Federally Qualified Health Center (FQHC) serving a multi-county area in southern Colorado with 34 service sites. We participate in Colorado Health Service Corps and National Health Service Corps loan repayment programs, with eligibility for up to $90,000 for providing care in rural communities.

Position Type: Full Time. Salary range: $106,666.40 – $159,999.60 per year.

Responsibilities

• Perform physical examinations, document physical findings in the EMR system (NextGen), review health history, and formulate a diagnosis based on the patient’s condition.
• Order, interpret, and evaluate diagnostic tests to identify and address clinical problems and healthcare needs.
• Prescribe pharmacological and non-pharmacological therapies; review and evaluate treatments for effectiveness.
• Collaborate with an integrated healthcare team to determine a comprehensive patient healthcare plan.
• Refer patients to other healthcare practitioners for consultations or specialized resources as appropriate.
• Educate patients and their families about treatment and healthcare needs.
• Assist in the oversight of support personnel with clinical guidance.
• Perform other duties commensurate with professional services as assigned by the employer.
