Location: Durango, CO 81303 (positions also available in Dove Creek, Cortez, and Pagosa Springs).

Position Type: Full Time. Salary Range: $109,990.40 – $138,590.40 annualized. Job Shift: Day.

Position Summary

The Nurse Practitioner will provide medical health services to patients in accordance with clinic protocols, collaborate with a multidisciplinary team to deliver comprehensive integrated health care services, and consult with staff regarding clinical interventions to achieve quality patient outcomes. Duties are tied to the privileges granted to the practitioner by the medical staff.

Responsibilities

• Perform appropriate history and physical examinations.
• Order and interpret diagnostic studies.
• Develop and implement treatment plans, including medications and prescriptions.
• Consult with physicians regarding referrals or need for higher level evaluation.
• Collaborate with clinic personnel and physicians.
• Participate in the development of integrated health care models and clinic protocols.
• Maintain patient confidentiality and follow security rules.
• Document care in the electronic medical record.

Required Qualifications

• Registered Nurse State of Colorado.
• Certified Nurse Practitioner State of Colorado.
• DEA licensure and Prescriptive Authority.
• ACLS Certification.
• National Acute Care Nurse Practitioner Certification when applicable.
• Formal training in the area of specialization.
• Experience in the area of practice as described by the sponsoring physician.
• Organized, self‑directed, with excellent interpersonal and communication skills.
• Adherence to confidentiality and security policies.

Required Skills

• Proficiency with electronic medical records and related systems.
• Ability to operate standard office equipment (calculator, copier, fax, multi‑line telephone).
• Strong organizational and prioritization skills in a fast‑paced environment.
• Current driver’s license and eligibility for commercial insurance.
• Knowledge of universal precautions and ability to report communicable disease exposure.
• Independent judgment and prescription management of psychotropic medications.
• Familiarity with treatment of co‑morbid substance use/abuse disorders, including ASAM levels of care.

Physical Demands

The role requires occasional lifting of up to 15 pounds, frequent walking, sitting, standing or kneeling, occasional climbing or balancing, stooping, and prolonged computer use. Ability to travel between Axis Health System locations is required.
