Job summary

Community of Hope is seeking a Family Nurse Practitioner to deliver primary and preventive care across the lifespan, closely collaborating with physicians, nurses, and care teams at the Conway Health & Resource Center in SW Washington, DC.

The position emphasizes care coordination, chronic disease management, and health promotion, with a strong focus on equity and compassionate, trauma-informed care for underserved populations.

Qualifications

• Completion of a Family Nurse Practitioner program as required.
• National Certification as Family Nurse Practitioner is required.
• Licensed to practice in the District of Columbia is required; MD licensure reimbursement information available for MD-licensed providers.
• BLS certification required; ACLS or BLS for Healthcare Provider equivalents acceptable.

Responsibilities

• Provides direct, team-based clinical care to patients within scope of practice.
• Provides virtual medical care and refers to in-person visits when needed.
• Refers patients to specialists and coordinates care as needed.
• Adheres to clinical and office policies and protocols.
• Maintains productivity numbers as set by Community of Hope.
• Precepts residents and students during rotations with supervisor approval.
• Ensures safe, secure, and healthy work environment per standards.
• Signs off on lab results, imaging, and referrals promptly; communicates with patients.
• Performs other duties as requested by supervisor.
