Job summary

Vivia Health in Chicago seeks a Family Nurse Practitioner to deliver high-quality, comprehensive primary care to adults in underserved communities. You will work with an interdisciplinary team to manage preventive care, disease management, and overall wellness.

You will diagnose, treat, and manage chronic conditions, perform minor procedures, and educate patients and families. Licensing in Illinois and national NP certification are required, with DEA license preferred.

Qualifications

• MSN or DNP required.
• 1-2 years of NP primary care experience.
• National NP certification (AANP or ANCC) required.
• Illinois license with active registration in good standing.
• DEA license preferred.
• Experience with mental health and substance use disorders.
• Strong teamwork and collaboration across specialties.
• Excellent communication and interpersonal skills.
• Proficient with EHR and healthcare software.
• Strong organizational skills and attention to detail.

Responsibilities

• Provide primary and preventive care to a diverse adult patient population.
• Conduct comprehensive health assessments and patient histories.
• Diagnose and treat acute and chronic illnesses, conditions, and injuries.
• Chronic disease management and patient education.
• Perform medication reconciliation and adjust regimens.
• Order, perform, and interpret diagnostic tests and lab work.
• Conduct minor procedures (suturing, wound care, incision, drainage).
• Educate patients on health management and prevention.
• Collaborate with physicians and other health professionals for comprehensive care.
• Maintain accurate EHRs and document interactions and outcomes.
• Participate in practice management and quality improvement initiatives.
• Supervise nursing staff, students, and interns.
• Clinic-based schedule Monday to Friday across multiple Chicago clinics.
