Solstice Health in Wisconsin seeks an experienced Family Practice Nurse Practitioner to join our Direct Primary Care team, delivering patient-centered care outside traditional insurance models.

You will provide comprehensive primary care, including acute and chronic disease management, preventive services, immunizations, and telemedicine. Work with an interdisciplinary team and contribute to continuous improvement in care delivery.

Qualifications

• Licensed as a Wisconsin Nurse Practitioner (active) and FNP board-certified.
• Experience in primary care or family practice.
• Strong clinical judgment and diagnostic skills.
• Excellent interpersonal and patient engagement abilities.
• Ability to work independently within a collaborative team and with EHR familiarity.

Responsibilities

• Deliver a full range of primary care services including check-ups, preventive care, and immunizations.
• Provide care for DPC patients both in-person and via telemedicine.
• Diagnose, treat, and manage acute and chronic conditions with a preventive focus.
• Collaborate with an interdisciplinary team and Medical Director for coordinated care.
• Maintain accurate, timely EHR documentation and records.

We are seeking an experienced Family Practice Nurse Practitionerwho is passionate about patient-centered care and eager to help revolutionize healthcare delivery outside the traditional insurance-based system.

Responsibilities

• Deliver a full range of acute, chronic, preventive, and alternative healthcare services, including routine check-ups, health-risk assessments, immunizations, screening tests, and personalized counseling on maintaining a healthy lifestyle
• Provide comprehensive care for Direct Primary Care (DPC) patients, utilizing clinic resources and materials
• Respond to patient calls, correspondence, and requests, including in-office visits and telemedicine, in alignment with the 24/7 unlimited access principles of Direct Primary Care
• Diagnose, treat, and manage acute and chronic medical conditions with a focus on prevention, longevity, and whole-person care
• Practice as a member of an interdisciplinary healthcare team, providing care through direct treatment, consultation, collaboration, coordination, and referrals as appropriate
• Utilize medical nutritionto promote health and prevent or manage conditions such as cancer, obesity, food allergies, kidney disease, cardiovascular disease, diabetes, and hypertension
• Develop and manage individualized treatment plans, including lifestyle optimization and weight management strategies
• Maintain accurate, complete, and timely patient records in the electronic health record (EHR) system
• Prepare and attend to all reports, documentation, and correspondence necessary or appropriate in connection with patient care; all such records remain the property of Solstice Health
• Protect the Company’s value by maintaining strict confidentiality of patient information and clinic operations
• Perform all additional duties reasonably necessary to maintain clinic operations and continuously improve professional skills
• Collaborate closely with the Medical Director and healthcare team to ensure high-quality, patient-centric care

Qualifications

• Active Wisconsin Nurse Practitioner license
• Board certification as a Family Nurse Practitioner (FNP)
• Proven experience in primary care or family practice
• Strong clinical judgment and diagnostic skills
• Excellent interpersonal, communication, and patient engagement abilities
• Ability to work independently while thriving in a collaborative, team-oriented environment
• Familiarity with electronic health record systems preferred
