POSITION SUMMARY
Primarily responsible for providing comprehensive primary health care services through the identification, management and/or referral of health problems and the maintenance of client health by means of preventive care and health promotion. Performs assignments that vary from routine to exceptional in nature with minimal guidance in accordance with clinic office practices and procedures. Complies with organization and department policies.

QUALIFICATIONS – ESSENTIAL SKILLS AND EXPERIENCE
- Graduate from an approved Nurse Practitioner school
- Nurse Practitioner certificate issued by the Arizona Board of Nursing
- Valid Arizona Registered Nurse license
- Valid Arizona Advanced Practice license
- Valid DEA certificate
- Current BLS
- Demonstrated understanding of clinical methods and techniques
- Ability to meet and deal effectively with clients, associates, and the general public
- Basic business English, spelling, and punctuation skills
- Basic computer efficiency
- Valid AZ driver’s license

- Competency in dealing with people of various cultures and social status
QUALIFICATIONS – NONESSENTIAL SKILLS AND EXPERIENCE
- Three (3) years of experience in community health
- Bilingual – English/Spanish

POSITION RESPONSIBILITIES / ESSENTIAL
- Health Care Delivery – Reports to the Chief Medical Officer. Provide high quality health care services including accessible and available health services; education of patients on diagnoses, treatment, preventive measures, and health care system navigation; consistent treatment based on clinical impressions and working diagnoses; accurate and complete electronic medical record entries in a timely manner; adequate and appropriate transfer of information for continuity of care; timely, courteous, and professional patient evaluation; completion of all required documentation for billing; promotion of good customer service; collaboration with other disciplines; advising management on performance deficiencies of medical auxiliary personnel; demonstration of knowledge of licensing standards for mid-level practice; assume the role and duties of other clinical personnel as necessary; rotate call for emergencies in evenings, weekends, and holidays as assigned; attend continuing medical education (CME) programs as paid for by Adelante Healthcare and required to maintain licensure; maintain credentialing and privileging in good standing.
QUALITY MANAGEMENT / RISK MANAGEMENT
- Demonstrate evidence of continuing professional growth through participation in quality improvement programs, peer review, medical record audits, continuing education, and review of protocols and procedures.
- Review utilization and quality monitoring results and participate in analysis of services.
- Communicate necessary information to the Chief Medical Officer regarding operations and suggestions for improvement.
- Be knowledgeable and adhere to all policies and procedures related to client services.
- Assure compliance with regulations set forth by the Department of Health and Human Services and other contracted health care plans.
COMMUNITY RELATIONS
- Represent Adelante Healthcare as requested in groups addressing health care needs of the community.
- Participate in planning and development of new programs and services by identifying specific community health needs.
- Assist individuals and the community in taking responsibility for prevention, promotion, maintenance, and restoration of health.
- Act as liaison and address community groups regarding Adelante Healthcare services.
- Understand role in the Disaster Plan and that safety is a condition of employment.
- Participate in health fairs, classes, and community events.
PROFESSIONAL NETWORKING
- Attend monthly provider meetings and combined staff meetings.
- Participate and complete required training.
- Demonstrate skills and behaviors consistent with Language of Caring.
- Contribute to training and proficiencies of clinical personnel, including medical students, in a culturally competent capacity.
- Provide coverage for other Adelante sites when necessary.
- Perform other related duties as assigned.

PHYSICAL DEMANDS AND WORK ENVIRONMENT
Physical demands include prolonged sitting, standing, walking, using hands to handle or feel objects, reaching, climbing steps, balancing, stooping, kneeling, crouching, coughing, talking, hearing, smelling, and managing stress. Frequent lifting of up to 25 pounds without assistance, occasionally up to 50 pounds with assistance. Vision requirements include close, distance, color, peripheral, and depth perception, and the ability to adjust focus.
Work environment includes exposure to weather conditions when traveling to clinic sites, moderate noise levels, and health hazards such as contagious diseases and bloodborne pathogens.
