Job summary

MyCHN is seeking a dedicated Whole Person Care Nurse Practitioner to join our nonprofit team serving the Greater Houston and Gulf Coast region. You will travel to clinics across Angleton, Alvin, Lake Jackson, Clute, and Freeport to deliver patient-centered primary care alongside an interdisciplinary team.

As a Family Nurse Practitioner, you will assess and manage care across ages, emphasizing holistic health, prevention, and education while advancing health outcomes in underserved communities.

Qualifications

  • Licensed Family Nurse Practitioner with APRN or PA recognition in Texas.
  • Master of Science in Nursing.
  • Certification in area of practice or eligibility for certification.
  • Active DEA license or eligibility to obtain within first month.
  • Strong clinical assessment, diagnostic, and patient management skills.
  • Ability to communicate with diverse populations.
  • Commitment to quality healthcare and whole-person care.
  • Ability to manage multiple priorities in a fast-paced environment.

Responsibilities

  • Assess, diagnose, and manage healthcare needs across the lifespan.
  • Obtain comprehensive health histories via interviews and exams.
  • Identify health risks and develop treatment plans based on findings.
  • Provide wellness, preventive care, and chronic disease management.
  • Counsel and educate patients on conditions and treatments.
  • Collaborate with physicians and other professionals for coordinated care.
  • Refer patients to specialty care when needed.
  • Maintain complete, timely patient documentation.
  • Ensure prompt follow-up on labs, referrals, and meds.
  • Support quality improvement and positive health outcomes.
  • Maintain licensure and continuing education requirements.
  • Perform other duties as assigned.

As a Whole Person Care Nurse Practitioner, you'll provide primary care services to patients of all ages while collaborating with an interdisciplinary team to deliver integrated, patient-centered care that improves health outcomes.

Your Responsibilities
  • Assess, diagnose, and manage the healthcare needs of patients across the lifespan.
  • Obtain comprehensive health histories through patient interviews, family discussions, and physical examinations.
  • Identify health risks and develop appropriate treatment plans based on clinical findings, diagnostic testing, and patient assessments.
  • Provide wellness assessments, preventive care, and chronic disease management.
  • Counsel and educate patients on health conditions, treatment plans, and health-promoting activities.
  • Collaborate with physicians and other healthcare professionals to deliver coordinated, whole-person care.
  • Refer patients with unstable or complex conditions to specialty care when appropriate.
  • Maintain complete, timely, and accurate patient documentation.
  • Ensure prompt follow-up regarding laboratory results, referrals, medication needs, and patient inquiries.
  • Support quality improvement initiatives and contribute to positive patient health outcomes.
  • Maintain professional licensure and continuing education requirements.
  • Perform other duties as assigned.
What You'll Need
  • Licensed Family Nurse Practitioner with recognition as an Advanced Practice Registered Nurse (APRN) or licensed Physician Assistant recognized by the Texas Medical Board
  • Master of Science in Nursing
  • Certification in area of practice or eligibility for certification
  • Active DEA license or the ability to obtain DEA certification within the first month of employment
  • Strong clinical assessment, diagnostic, and patient management skills
  • Ability to effectively communicate with individuals from diverse socioeconomic backgrounds
  • Commitment to quality healthcare, patient advocacy, and whole-person care
  • Ability to manage multiple priorities in a fast-paced healthcare environment
  • Strong collaboration and teamwork skills
Preferred Qualifications
  • ACL s Certification
  • Experience in primary care, family practice, or acute care settings
  • Experience working in a nonprofit healthcare environment
  • Supervisory or leadership experience
