Job summary

MyCHN is seeking a Whole Person Care Nurse Practitioner to provide primary care across the lifespan in a multi-clinic setting. You will travel to Angleton, Alvin, Lake Jackson, Clute, and Freeport to deliver integrated, patient-centered care with an interdisciplinary team.

The role emphasizes holistic care, health promotion, and collaboration with physicians to improve outcomes in underserved communities. A Texas-licensed APRN/PA with MSN and DEA credentials is required.

Qualifications

• Licensed Family Nurse Practitioner with recognition as an APRN or licensed Physician Assistant recognized by the Texas Medical Board.
• Master of Science in Nursing (MSN) required.
• Certification in area of practice or eligibility for certification.
• Active DEA license or ability to obtain DEA certification within the first month.

As a Whole Person Care Nurse Practitioner, you'll provide primary care services to patients of all ages while collaborating with an interdisciplinary team to deliver integrated, patient-centered care that improves health outcomes.

This position requires traveling to the following clinics:

• Angleton
• Alvin
• Lake Jackson
• Clute
• Freeport

Your Responsibilities

• Assess, diagnose, and manage the healthcare needs of patients across the lifespan.
• Obtain comprehensive health histories through patient interviews, family discussions, and physical examinations.
• Identify health risks and develop appropriate treatment plans based on clinical findings, diagnostic testing, and patient assessments.
• Provide wellness assessments, preventive care, and chronic disease management.
• Counsel and educate patients on health conditions, treatment plans, and health-promoting activities.
• Collaborate with physicians and other healthcare professionals to deliver coordinated, whole-person care.
• Refer patients with unstable or complex conditions to specialty care when appropriate.
• Maintain complete, timely, and accurate patient documentation.
• Ensure prompt follow-up regarding laboratory results, referrals, medication needs, and patient inquiries.
• Support quality improvement initiatives and contribute to positive patient health outcomes.
• Maintain professional licensure and continuing education requirements.
• Perform other duties as assigned.

What You'll Need

• Licensed Family Nurse Practitioner with recognition as an Advanced Practice Registered Nurse (APRN) or licensed Physician Assistant recognized by the Texas Medical Board
• Master of Science in Nursing
• Certification in area of practice or eligibility for certification
• Active DEA license or the ability to obtain DEA certification within the first month of employment
• Strong clinical assessment, diagnostic, and patient management skills
• Ability to effectively communicate with individuals from diverse socioeconomic backgrounds
• Commitment to quality healthcare, patient advocacy, and whole-person care
• Ability to manage multiple priorities in a fast-paced healthcare environment
• Strong collaboration and teamwork skills

Preferred Qualifications

• ACL s Certification
• Experience in primary care, family practice, or acute care settings
• Experience working in a nonprofit healthcare environment
• Supervisory or leadership experience
