Responsibilities include reviewing histories, ordering labs, developing treatment plans, prescribing medications, coordinating referrals, and documenting in the practice system.

Qualifications

• Family Nurse Practitioner with SC licensure and board certification.
• Strong foundation in primary care, preventive and chronic disease management.
• Ability to provide broad-scope family practice services including acute conditions.
• Graduate degree with prescriptive authority in SC.
• Current Basic Life Support (BLS) certification.
• Outpatient/family practice experience; Direct Primary Care or value-based care a plus.
• Proficient with EHR and clinical documentation; evidence-based practice.
• Excellent communication and patient-centered approach.

Responsibilities

• Review patient histories, order and interpret labs and diagnostics.
• Develop and implement evidence-based treatment plans.
• Prescribe medications and coordinate referrals when needed.
• Collaborate with a small clinical team and maintain timely documentation.
• Support process improvements for accessible, transparent care for patients and employers.
• Provide comprehensive primary care services within DPC model.
