Job summary

PeopleOne Health is seeking a Family Nurse Practitioner to provide primary and preventive care for patients aged 2 and up. You will assess, diagnose, treat, and educate to improve health outcomes, while prescribing medications and coordinating care within an interdisciplinary team.

You will work autonomously with physicians and maintain documentation in EHRs, ensuring compliance with state and federal guidelines. This role emphasizes preventive care and wellness in a direct primary care model.

Qualifications

• Master’s or Doctorate degree in Nursing from an accredited program required.
• Current state licensure as a CRNP required.
• National certification in a relevant specialty (AANP/ANCC) required.
• Current DEA registration for prescribing medications required.
• BLS and ACLS certifications required.

Responsibilities

• Conducts comprehensive health assessments and diagnostic evaluations.
• Diagnoses and manages acute and chronic illnesses with treatment plans.
• Prescribes medications and non-pharmacologic therapies.
• Orders and interprets diagnostic tests and EKGs.
• Provides health counseling and wellness education for patients and families.
• Performs procedures within scope of practice.
• Collaborates with specialists for referrals and care continuity.
• Maintains accurate EHR documentation per regulations.
