Role Description

The Family Nurse Practitioner role is a full-time, on-site position based in Charleston, SC. This role provides comprehensive primary care services, including health assessments, chronic disease management, acute and urgent care visits, preventive care, and wellness counseling for individuals and families. The practitioner will manage a patient panel within the DPC model, focusing on longer visits, direct communication, and relationship-centered care without insurance billing. Daily responsibilities include reviewing patient histories, ordering and interpreting labs and diagnostics, developing evidence-based treatment plans, prescribing medications, and coordinating referrals when necessary. The role also involves collaborating with a small clinical team, maintaining accurate and timely documentation in the practice’s systems, and contributing to process improvements that support accessible, transparent care for patients and employer groups.

Qualifications

• Clinical practice expertise as a Family Nurse Practitioner, with current Nurse Practitioner licensure and board certification.
• Strong foundation in Nursing and Primary Care, including preventive services, chronic disease management, and family-centered care.
• Ability to provide broad-scope Family Practice services, including evaluation and treatment of common acute and minor urgent conditions.
• Graduate degree from an accredited nursing program (MSN, DNP or equivalent) and prescriptive authority in South Carolina.
• Current Basic Life Support (BLS) certification.
• Experience in outpatient or family practice settings; familiarity with Direct Primary Care or value-based care models is a plus.
• Proficiency with electronic health records, strong clinical documentation skills, and commitment to evidence-based practice.
• Excellent communication and interpersonal skills, with a patient-centered, culturally sensitive approach and the ability to work collaboratively in a small team environment.
