Qualifications

• Master's degree in nursing required.
• Graduate from NP, CNS or midwife program.
• Board-certified APRN; family-certified preferred.
• Active nursing license; specialty certification.
• 3–4 years of clinical nursing in family practice, geriatric, occupational health or related setting.

Responsibilities

• Provides episodic and chronic care, screening, and education to patients in the onsite clinic.
• Collaborates with wellness service colleagues and adheres to current evidence-based guidelines.
• Floats across clinics and handles patient registration and copay processes.
• Collaborates with physicians to plan comprehensive care and advises on cost and quality benefits.
• Educates patients and families to promote health and prevention.
