True North Consultants, Inc. is hiring a Family Nurse Practitioner for a premier physician-led group with locations across Long Island.

This role is 100% outpatient with a focus on primary care, preventive, acute and chronic disease management, in a flexible, full-time or part-time schedule with no on-call requirements. The FNP will collaborate with physicians, refer to specialists as needed, ensure high-quality patient care, and maintain thorough EMR documentation.

Qualifications

• Must hold NYS RN and NP licenses, plus BSN and MSN.
• Board-certified with BLS.
• 8+ years of NP experience.
• Strong diagnostic, communication, and interpersonal skills.
• Entrepreneurial mindset with ability to grow a patient base.
• Experience with EMRs and value-based care metrics is a plus.

Responsibilities

• Deliver outpatient primary care, including preventive and chronic disease management.
• Refer patients to appropriate specialty services.
• Educate patients on diagnoses, treatment options, and prevention.
• Maintain accurate EMR documentation.
• Collaborate with physicians, specialists, and care teams.
• Provide clinical oversight to nurses, technologists, and support staff.
• Stay compliant with HIPAA, NYS DOH, CMS, and payer guidelines.
• Participate in population health initiatives and quality improvement.
• Contribute to developing a sustainable patient panel.

A well-established, physician-led group with locations throughout Long Island, is currently seeking to hire a Family Nurse Practitioner. This practice has built a stellar reputation for clinical excellence, patient satisfaction, and multidisciplinary collaboration. 100% outpatient.

Key Responsibilities:

• Serve as a key clinical provider in the delivery of high-quality care.
• Provide direct patient care in an outpatient primary care setting, including preventive, acute, and chronic disease management.
• Refer patients to appropriate specialty services as needed.
• Provide clear communication and education to patients regarding diagnoses, treatment options, and preventive strategies.
• Maintain thorough and accurate documentation within the Electronic Medical Record (EMR) system.
• Collaborate with referring physicians, specialists, and care teams to ensure seamless patient care.
• Provide clinical oversight to nurses, technologists, and support staff within the practice.
• Stay compliant with HIPAA, NYS DOH, CMS, and relevant payer guidelines.
• Participate in population health initiatives and support ongoing quality improvement projects.
• Contribute to the development of a sustainable patient panel aligned with practice growth objectives.
