Job summary

Urban Health Plan in the Bronx, NY is seeking a Family Nurse Practitioner to join our primary care team. The FNP will provide comprehensive primary care services to a diverse patient population, collaborating with a multidisciplinary team to manage exams, tests, diagnoses, treatment plans, and referrals.

The ideal candidate has completed an accredited NP program, holds NY State licensure with CDS/DEA and BLS, and demonstrates strong communication and a commitment to community health.

Qualifications

• Completion of an approved/accredited educational program for Nurse Practitioners.
• Valid NY State license, CDS, DEA, and BLS.
• Ability to effectively communicate, follow established procedures and protocols.

Job Description:

We are looking forFamily Nurse Practitioners to provide primary care services as part of our primary care team. The FNP will work with a multi-disciplinary team to care for and provide medical services to patients, including examining/conducting tests; analyze records, reports, and test results; to prevent and/or diagnose illness, disease or injury; prescribe/administer treatment, therapy, medication, vaccination, and other specialized medical care to treat or prevent illness, disease, and/or injury; explain procedures and test results on prescribed treatment plan; manage labs, referrals. The FNP will collaborate with other physician assistants, nurse practitioners, physicians, and other support staff as part of the clinical team. Perform other job-related duties as required by departmental needs/demands.

Experience and Skills:

• Completion of an approved/accredited educational program for Nurse Practitioners
