Job summary

Urban Health Plan in the Bronx, NY is seeking an Adult Gerontology Nurse Practitioner to join our primary care team. You will deliver patient-centered care to diverse communities, perform exams, order tests, and coordinate treatment within a multidisciplinary group.

The AGNP will work with physicians, physician assistants, and support staff to provide preventive care, manage chronic conditions, vaccines, referrals, and patient education in a high-quality, community-focused environment.

Qualifications

• Completion of an approved/accredited educational program for Nurse Practitioners.
• Valid NY State license, CDS, DEA, and BLS.
• Ability to effectively communicate, follow established procedures and protocols.
• Understanding of and passion for Community Healthcare is preferred.

Responsibilities

• Provide primary care services as part of our primary care team.
• Examine patients, conduct tests, analyze results to diagnose and treat.
• Prescribe/administer treatment, manage labs, referrals, and follow-up.
• Collaborate with physicians, PAs, NPs, and support staff to coordinate care.

Job Description:

We are looking for Adult Geriatric Nurse Practitioners to provide primary care services as part of our primary care team. The AGNP will work with a multi-disciplinary team to care for and provide medical services to patients, including examining/conducting tests; analyze records, reports, and test results; to prevent and/or diagnose illness, disease or injury; prescribe/administer treatment, therapy, medication, vaccination, and other specialized medical care to treat or prevent illness, disease, and/or injury; explain procedures and test results on prescribed treatment plan; manage labs, referrals. The AGNP will collaborate with other physician assistants, nurse practitioners, physicians, and other support staff as part of the clinical team. Perform other job-related duties as required by departmental needs/demands.

Experience and Skills:

• Completion of an approved/accredited educational program for Nurse Practitioners
• Valid NY State license, CDS, DEA, and BLS
• Ability to effectively communicate, follow established procedures and protocols
• Understanding of and passion for Community Healthcare is preferred
