Job summary

Pharmacy1st Health is building the clinical infrastructure for rural America by transforming independent pharmacies into integrated care delivery hubs. You will practice with autonomy, purpose, and support while helping launch a modern, community based care model that blends primary care, prevention, and technology enabled access.

This is an opportunity to be a founding clinician with real ownership in how the clinic operates and grows, delivering care at the top of your license and building

Qualifications

• Must have a Master’s or Doctoral degree in Nursing and be licensed in the state.
• Must hold national FNP certification such as FNP-BC or FNP-C.
• Experience in primary care, community health, or rural health preferred.
• Competence with EHR systems and telehealth platforms required.

Responsibilities

• Deliver comprehensive primary care with focus on prevention and continuity.
• Serve as clinical leader and trusted partner to patients and families.
• Collaborate with care team including pharmacists, care coordinators, and virtual care partners.
• Design workflows, care standards, and patient experience from the ground up.
• Contribute ideas and leadership as the clinic scales and evolves.

Pharmacy1st Health is building the clinical infrastructure for rural America by transforming independent pharmacies into integrated care delivery hubs. Our model combines primary care, pharmacy services, and chronic disease management under one roof, creating a new, lower-cost site of care for underserved communities.

We operate across multiple states and are rapidly expanding a network of rural clinics co-located with pharmacies. Our goal is to deliver scalable, high-quality care while improving access, outcomes, and cost efficiency.

From day one, you will play a central role in shaping how care is delivered in a community where access truly matters. You will practice with autonomy, purpose, and support while helping launch a modern, community based care model that blends primary care, prevention, and technology enabled access. You will also help influence the growth of the care team by participating in the recruitment and selection of future clinical roles, ensuring we build a team aligned in values, quality, and commitment to the community.   Who You Are

  • A Family Nurse Practitioner with a Master’s or Doctoral degree in Nursing
  • Licensed in the job's respective state with national certification such as FNP-BC or FNP-C
  • Experienced in primary care, community health, or rural health environments
  • Confident using EHR systems and telehealth platforms
  • Entrepreneurial, adaptable, and excited by building something meaningful
  • Deeply committed to patient centered, relationship driven care
