Position Summary
We are seeking a compassionate, skilled Family Nurse Practitioner (FNP) to join a small, independently owned, nurse-practitioner-led family practice. This role is ideal for an NP who values autonomous practice, relationship-based care, and whole-person health. This role is ideal for an individual who wants to be a part of the decision-making process, team development and practice growth. The Nurse Practitioner will provide comprehensive primary care across the lifespan, focusing on prevention, chronic disease management, and long-term partnerships with patients and families. Our practice prioritizes time with patients, continuity of care, and clinical decision-making rooted in evidence-based practice and individualized patient goals.
Key Responsibilities
Clinical Care
- Provide comprehensive primary care services for patients of all ages, including:
- Health maintenance and preventive care
- Acute illness and injury evaluation and treatment
- Chronic disease management (e.g., diabetes, hypertension, asthma, mental health conditions)
- Women's and men's health services
- Geriatric care
- Perform physical exams, assessments, diagnostic evaluations, and treatment planning
- Prescribe medications and therapies within scope of practice and state regulations
- Interpret lab results, imaging, and other diagnostic studies
- Coordinate care with specialists and community resources as needed
Whole-Person & Relationship-Based Care
- Develop long-term, trusting relationships with patients and families
- Address physical, emotional, social, and lifestyle factors impacting health
- Engage patients in shared decision-making and individualized care planning
- Provide patient education focused on empowerment, prevention, and self-management
- Support continuity of care through follow-up, care coordination, and accessibility
Practice & Collaboration
- Practice independently while collaborating with other healthcare professionals as appropriate
- Participate in practice development, quality improvement, and workflow optimization
- Maintain accurate, timely, and compliant medical documentation
- Uphold ethical, professional, and regulatory standards of advanced practice nursing
Qualifications
- Active, unrestricted Nurse Practitioner license in the state of practice
- Family Nurse Practitioner (FNP-C or FNP-BC) certification
- Eligible for independent practice per state regulations
- DEA registration (or ability to obtain)
- Minimum 1-2 years of primary care experience preferred (new graduates with strong primary care interest considered)
- Strong clinical judgment, communication, and interpersonal skills
- Commitment to patient-centered, holistic, and relationship-driven care
