Job summary

Gateway Community Health Centers in Gatesville, NC is seeking a passionate Family Nurse Practitioner to deliver high-quality primary care to patients across the lifespan. You will provide comprehensive assessments, manage acute and chronic conditions, and educate patients while collaborating with a multidisciplinary team.

The role requires FNP certification (AANP/ANCC), NC licensure, and an MSN or DNP. DEA registration or eligibility is needed, with experience in primary care or FQHC settings

Qualifications

• FNP certification from AANP or ANCC is required.
• Active or eligible NC NP licensure.
• MSN or DNP in Nursing from an accredited program.
• DEA registration or eligibility to obtain.

Responsibilities

• Provide comprehensive primary care to patients across the lifespan, including pediatrics, adults, and geriatrics.
• Conduct thorough patient assessments and develop individualized care plans.
• Diagnose and manage acute and chronic conditions (diabetes, hypertension, respiratory illness).
• Order and interpret tests and imaging to guide decisions.
• Prescribe medications per evidence-based guidelines and state regulations.
• Educate patients on prevention, health promotion, and chronic disease management.
• Collaborate with physicians and care team for coordinated, whole-person care.
• Maintain accurate documentation in the EHR (eClinicalWorks or similar).

Responsibilities:

Provide comprehensive primary care services to patients across the lifespan, including pediatric, adult, and geriatric populations

Conduct thorough patient assessments, physical exams, and develop individualized care plans

Diagnose and manage acute and chronic conditions including diabetes, hypertension, and respiratory illness

Order and interpret diagnostic tests, labs, and imaging to guide clinical decision-making

Prescribe medications and therapies in accordance with evidence-based guidelines and state regulations

Provide patient education on disease prevention, health promotion, and chronic disease management

Collaborate with physicians, specialists, and care team members to ensure coordinated, whole-person care

Maintain accurate and timely documentation in the Electronic Health Record (EHR) system

Requirements:

Current Family Nurse Practitioner (FNP) certification from AANP or ANCC

Active or eligible Nurse Practitioner licensure in the state of North Carolina

Master's or Doctoral degree in Nursing (MSN or DNP) from an accredited program

DEA registration and prescriptive authority (or ability to obtain)

Experience in primary care, community health, or federally qualified health center (FQHC) settings preferred

Proficiency with Electronic Health Records (EHR); experience with eClinicalWorks a plus

Strong interpersonal and communication skills with a patient-centered approach
