HHM Health is seeking a Family Nurse Practitioner to join our Family Medicine Healthcare team in the Dallas area. You will provide comprehensive primary care across the lifespan, collaborating with physicians and staff to deliver high-quality, patient-centered services.

Qualifications

• BSN/MSN and current Texas APRN license.
• Active or eligible NP licensure in Texas.
• National board FNP certification a plus.
• Experience in primary care is preferred.

Responsibilities

• Conduct comprehensive patient assessments including histories and exams.
• Diagnose and treat acute and chronic illnesses across ages.
• Order, perform and interpret diagnostic tests.
• Develop and implement individualized treatment plans.
• Prescribe medications per state regulations and scopes of practice.
• Provide preventive care, immunizations, and health education.
• Manage chronic conditions (diabetes, hypertension, obesity, etc.).
• Counsel on disease prevention, nutrition, lifestyle changes.
• Collaborate with providers to ensure continuity of care.

We are seeking a Family Nurse Practitioner with a passion for service excellence to join our Family Medicine Healthcare team in the Kaufman area. The Family Nurse Practitioner (FNP) provides comprehensive primary healthcare services to patients across the lifespan, including assessment, diagnosis, treatment, and preventive care. The FNP collaborates with physicians, nurses, and interdisciplinary healthcare teams to deliver high-quality, patient-centered care while promoting wellness and disease prevention.

What you need to succeed

• BSN/MSN from an accredited school of nursing or equivalent; current advanced practice license to practice in the state of Texas
• Registered and have current DEA, DPS and other such certificates to legallyoperateas apractitioner in the State of Texas or be able to obtain
• National board certification as a Family Nurse Practitioner (FNP-BC or FNP-C) is a plus
• Current and unrestricted Nurse Practitioner licensure in the state of Texas
• Experience in primary care, family medicine, community health, or outpatient settings preferred.
• Excellent patient engagement and communication skills
• Experience in community health centers and/or non-profits is preferred
• Experience serving multicultural and multilingual patient populations
• Experience with eClinicalWorks or similar
• MS Office Suite skills (Outlook, Teams, Word,PowerPointand Excel)
• Have a servant heart with the desire to make a positive impact
