Job summary

HHM Health in Kaufman, TX is seeking a Family Nurse Practitioner to join our Family Medicine team. The FNP will provide comprehensive primary care across the lifespan, collaborate with physicians and care teams, and deliver patient-centered preventive services.

The ideal candidate will hold a Texas APRN license, FNP certification, and experience in outpatient settings, with strong patient communication skills and a commitment to compassionate care for diverse populations.

Qualifications

• BSN/MSN from an accredited school; advanced practice license in Texas.
• Current DEA/DPS certificates to practice in Texas or ability to obtain.
• FNP-BC or FNP-C certification is a plus.
• Current unrestricted Texas NP licensure; primary care experience preferred.
• Experience with diverse, multicultural populations and outpatient settings.

Responsibilities

• Conduct comprehensive patient assessments, including medical histories and physical exams.
• Diagnose and treat acute and chronic illnesses across all ages.
• Order, perform, and interpret diagnostic tests and labs.
• Develop and implement individualized treatment and care plans.
• Prescribe medications and therapies per state regulations.
• Provide preventive care, immunizations, screenings, and health education.
• Manage chronic conditions like diabetes and hypertension.
• Counsel on disease prevention, nutrition, lifestyle, and wellness.
• Collaborate with physicians and staff to ensure continuity of care.

We are seeking a Family Nurse Practitioner with a passion for service excellence to join our Family Medicine Healthcare team in the Kaufman area. The Family Nurse Practitioner (FNP) provides comprehensive primary healthcare services to patients across the lifespan, including assessment, diagnosis, treatment, and preventive care. The FNP collaborates with physicians, nurses, and interdisciplinary healthcare teams to deliver high-quality, patient-centered care while promoting wellness and disease prevention.

• Conduct comprehensive patient assessments, including medical histories and physical examinations.

• Manage chronic conditions such as diabetes, hypertension, asthma, and obesity.
• Counsel patients and families on disease prevention, nutrition, lifestyle modifications, and wellness strategies.
• Collaborate with healthcare providers and staff to ensure continuity and quality of care.

What you need to succeed

• BSN/MSN from an accredited school of nursing or equivalent; current advanced practice license to practice in the state of Texas
• Registered and have current DEA, DPS and other such certificates to legallyoperateas apractitioner in the State of Texas or be able to obtain
• National board certification as a Family Nurse Practitioner (FNP-BC or FNP-C) is a plus
• Current and unrestricted Nurse Practitioner licensure in the state of Texas
• Experience in primary care, family medicine, community health, or outpatient settings preferred.
• Excellent patient engagement and communication skills
• Experience in community health centers and/or non-profits is preferred
• Experience serving multicultural and multilingual patient populations
• Experience with eClinicalWorks or similar
• MS Office Suite skills (Outlook, Teams, Word,PowerPointand Excel)
• Have a servant heart with the desire to make a positive impact
