POSITION SUMMARY

A full time, non-exempt position responsible for administering, maintaining, and expanding a program of primary (and in selected instances, secondary care) and preventive medical care in a community health center setting.

The Family Medicine Nurse Practitioner will be an experienced ARNP, who will act within their professional boundaries to provide care for patients with initial history taking, clinical assessment, diagnosis, treatment, and evaluation of care plans. They will demonstrate safe, clinical decision-making and expert care for patients within the practice. The provider will work collaboratively with a multi-disciplinary team to meet the needs of patients, supporting the delivery of care according to JFS policy and procedures. The provider will build effective working relationships with other health care professionals. The provider will work with a team of clinical support staff to deliver excellent primary health care services within the scope of a medical home model.

What You'll Do
1. Management of Patient Health
  • 1.1. To deliver a high standard of patient care as a Family Medicine Nurse Practitioner, using advanced clinical skills, and a broad and in-depth theoretical knowledge base.
  • 1.2. Provide a first point of contact within the practice for patients presenting with undifferentiated, undiagnosed problems, making use of skills in history taking, physical examination, problem-solving and clinical decision-making, to establish a diagnosis and management plan.
  • 1.3. To prioritize health problems and intervene appropriately, including initiation of effective emergency care.
  • 1.4. Initiate necessary invasive and non-invasive diagnostic tests or investigations and interpret findings/reports.
  • 1.5. To prescribe safe, effective, and appropriate medication using evidence-based guidelines and recommendations
  • 1.6. Provide safe, evidence-based, cost-effective, individualized patient care.
  • 1.7. Promote health and well-being using health promotion, health education, screening and therapeutic communication skills.
  • 1.8. To encourage and support patients’ ability to self-manage chronic conditions and to engage in preventive health measures.
  • 1.9. To clearly communicate the health status of the patient using appropriate terminology, technology, and format.
  • 1.10. To work within the practice guidelines, policies, and protocols.
  • 1.11. To maintain complete, accurate and timely medical records, utilizing electronic health record software
2. PROFESSIONAL ROLE
  • 2.1. To promote personal development and clinical excellence.
  • 2.2. Working with the clinical team to respond to the changing healthcare needs of the patient population.
  • 2.3. To participate in multi-disciplinary education.
  • 2.4. Participate in continuing professional development opportunities to ensure that up-to-date evidence-based knowledge and competence is maintained in all areas relevant to the scope of practice.
  • 2.5. Participate in organizational decision making, interpreting variations in outcomes and use data from information systems to improve practice.
  • 2.6. Monitor the effectiveness of clinical judgment through quality assurance strategies, such as peer review and clinical quality metrics.
  • 2.7. Participate in and support the development of new services within the practice.
3. TEAM ROLE
  • 3.1. To lead a primary care team, utilizing the methods of the medical home model.
  • 3.2. To support other healthcare team members and to accept referrals from other team members.
  • 3.3. To value all team members.
  • 3.4. To delegate appropriate tasks.
  • 3.5. To support the delivery of health and safety protocols, as agreed within the practice.
  • 3.6. To support, uphold and deliver all elements of clinical quality guidelines within the practice.
4. ORGANIZATIONAL
  • 4.1. To complete records, audits, reports and respond to appropriate questions and requests.
  • 4.2. To attend meetings as requested.
  • 4.3. Support effective communication channels between all JFS team members.
  • 4.4. Develop a sound understanding of the practice EMR.
  • 4.5. To inform the CMO of difficulties or problem areas when they arise.
  • 4.6. Practice fair and economical utilization of health care resources within and outside of the practice.
  • 4.7. To contribute to practice strategy and developments when requested.
  • 4.8. To maintain clinical practice within quality guidelines and support the QI Committee Initiatives.
  • 4.9. To mitigate risk within the practice by working to prevent sentinel events using proper infection control and patient safety strategies.
  • 4.10. Participate in the analysis of patient satisfaction data, as a means of quality improvement within the practice.
  • 4.12. To actively participate in the delivery of productivity and revenue targets as set by JFS’ chief officers.

  • 6.1. To always work within the highest levels of standards commensurate with the practice of medicine.
7. Health and Safety
  • 7.1. The physician must comply with all JFS health and safety policies, by following safe working procedures and following incident reporting protocols. Specifically, they must:
  • Comply with all safety instructions.
  • Use equipment and facilities in a proper and safe manner.
  • Refrain from the wilful misuse of the position or interference with protocols that are developed in the interest of health and safety.
  • Report any hazards and defects present in the clinical environment.
  • Report any accidents or incidents in the clinical environment, completing all appropriate forms as soon as possible.
8. Rules and Standing Orders
  • 8.1. The physician is required to comply with the rules, regulations, standing orders and financial instructions set forth by JFS policies.

Qualifications
  • Graduate of an accredited nurse practitioner program
  • Nurse Practitioner Certification in Family Medicine
  • Current Texas Medical License
  • Current DEA License
  • Current certification in Basic Life Support (BLS)
  • At least 1-2 years of experience as a nurse practitioner in an outpatient setting is required.
