Job summary

Canopy Children's Solutions in Meridian, Mississippi is seeking a Family Navigator to coordinate services for children and families. You will collaborate with families, use the NCFAS and CANOPY assessments, and create a plan of care that connects local resources to strengthen families.

Responsibilities include maintaining documentation within 48 hours, adhering to trauma-informed care, developing the Family Map with team support, providing transportation as needed, and supervising family

Qualifications

• Bachelor’s degree in social work or related field is required.
• Minimum 2 years’ experience working with children and families preferred.
• Must be able to work evenings and weekends as needed.
• Must be 21+ with valid Mississippi driver license and auto insurance.

Responsibilities

• Maintain CPR and first aid certification current.
• Engage families using the LINK model to develop a family care plan.
• Create a resource atlas with local resources for families.
• Ensure trauma-informed care and fidelity to the Link model.
• Develop and maintain the Family Map with team support.
