Job summary

Bridges Healthcare, Inc. in New Haven is seeking a Family Navigator IPV-FAIR to support caregivers involved with intimate partner violence.

The Family Navigator provides home-based services, case management, safety planning, and psychoeducation to strengthen families in the greater New Haven area. You will work with clinicians, manage records with strict confidentiality, and participate in regular supervision.

Qualifications

  • Bachelor's degree in a human services field, OR high school diploma with at least three (3) years extensive experience providing direct services to families with complex needs.
  • Understanding of trauma, child abuse prevention and treatment, family violence, and systems advocacy preferred.
  • Experience with case management and ability to work with diverse individuals.
  • Effective verbal and written communication skills.
  • Ability to work collaboratively as part of a team. Must have own insured vehicle in working order.
  • Valid driver's license, with access to reliable transportation on a consistent basis.

Responsibilities

  • Meet with clients in homes, the community, or the agency office to provide a range of supportive and case management services to caregivers in families experiencing domestic violence.
  • Provide trauma-informed, culturally responsive, and client-centered services.
  • Services include case management, safety planning, psychoeducation, advocacy, skill-building, referrals and service linkages.
  • Foster therapeutic alliances and maintain a non-judgmental stance with DV-impacted families.
  • Facilitate family engagement and use motivation-enhancement strategies.
  • Assist families in removing barriers like transportation.
  • Attend case reviews with DCF and maintain collaborative relationships with DCF staff.
  • Maintain complete records and documentation as required.
  • Attend supervision with IPV-FAIR Program Director.

Job description

Bridges Healthcare, Inc., has an immediate opening for a Family Navigator IPV-FAIR (Intimate Partner Violence - Family Assessment Intervention Response) in our Family Centered Services Division in New Haven.

Salary: $27.80 - $28.68 per hour, commensurate of experience

PURPOSE and POSITION OVERVIEW

The IPV-FAIR Family Navigator works in partnership with the assigned Clinician to support caregivers who have been involved with intimate partner violence. The Family Navigator provides primarily home-based services to families in the greater New Haven area and offers an array of supportive services which include case management, safety planning, psychoeducation, and other services as needed. IPV-FAIR services support caregivers in creating safer homes for their children and strengthening parent-child relationships.

RESPONSIBILITIES:
  • Meet with clients in their homes, the community, or the agency office to provide an array of supportive and case management services to all caregivers in families experiencing domestic violence.
  • Provide services which are trauma-informed, culturally responsive, and client-centered.
  • Services provided to families include case management, safety planning, psychoeducation, advocacy, skill-building, referrals and service linkages.
  • Foster therapeutic alliances and maintain a non-judgmental stance with all caregivers from DV-impacted families, including those who have experienced abuse and those who have caused harm.
  • Facilitate family engagement and employ motivation enhancement strategies.
  • Assist families in eliminating barriers such as transportation.
  • Maintain flexible work hours to accommodate family needs, including occasional evenings as needed, with late hours being flexed out elsewhere during pay period.
  • Attend regularly scheduled case reviews with DCF and maintain collaborative work relationships with DCF staff assigned to client families.
  • Attend all program, agency and other required meetings.
  • Complete all record keeping and documentation as required by the agency and its funding sources.
  • Attend regularly scheduled supervision (one hour weekly) with IPV-FAIR Program Director.
  • Maintain timely and complete individual case files and strict confidentiality.
  • Perform any other reasonably related duties as assigned by the IPV-FAIR Program Director.
