Job summary

Qualifications

• Bachelor's degree from an accredited college or university in a social-services related field.
• Minimum of two years of experience in case management or related field experience is required.

Responsibilities

• Provide one-on-one coaching around financial goals and connect families to resources.
• Regularly evaluate activities and progress toward goals and provide incentives as appropriate.
• Establish professional, supportive relationships with participants.
• Ensure participants understand program guidelines and stay focused on self-sufficiency goals.
• Document participant files and complete onboarding and assessments within guidelines.
• Coordinate transportation for appointments and activities within program parameters.
• Collaborate with supervisors and partners to adjust efforts as needed.
• Develop partnerships with public and private agencies to support participants.
• Provide facilitation for participant community meetings and trainings.
• Complete ongoing trainings as required.
• Perform other duties as assigned.
