Qualifications

• Board Certification or Eligibility in Family Medicine required
• CA Medical License active
• Completion of accredited Family Medicine Residency Program
• Spanish-speaking skills preferred
• Experience with HCC coding, HEDIS, and CMS quality measures preferred

• Provide comprehensive primary care across the lifespan.
• Collaborate with an interdisciplinary team to coordinate patient care.
• Build long-term patient relationships within a collaborative practice.
