Job summary

Praxis Health, Inc. is seeking an experienced Family Medicine Physician to join Advanced Primary Care (APC) Eugene. You’ll build your own patient panel within a growing, value-based primary care model focused on prevention, chronic disease management, and meaningful patient relationships.

You will work with a close-knit team and contribute ideas as the model evolves, enjoying collaboration and a supportive leadership culture that values problem-solving and innovation in care delivery.

Qualifications

• MD or DO with Board Certification/Board Eligibility in Family Medicine.
• Active or eligible for Oregon medical licensure.
• Minimum of 2 years post-training primary care experience; 5+ years strongly preferred.
• Strong foundation in comprehensive adult primary care and chronic disease management.
• Experience with routine outpatient procedures.
• Women’s health experience highly valued.
• Interest in preventive medicine, population health, and value-based care.

Responsibilities

Advanced Primary Care (APC) - Eugene, part of Praxis Health, is seeking an experienced Family Medicine Physician to join our growing Eugene practice. APC is reimagining what primary care can look like through an innovative, value-based model focused on access, prevention, population health, and meaningful patient relationships. Rather than simply treating illness as it arises, our providers proactively manage chronic conditions, close care gaps, and partner with patients to help them stay healthy and out of the hospital.

APC Eugene is growing, which means you'll have the opportunity to build your own patient panel while contributing your experience and ideas as we continue developing the model.
