Praxis Health – Advanced Primary Care (APC) in Eugene, Oregon, seeks a Family Medicine Physician to join a value‑based model focused on access, prevention, population health and meaningful patient relationships.

Benefits include a guaranteed base salary of $275,000–$300,000 DOE, 32 patient‑facing hours plus admin time, sign‑on bonus, comprehensive benefits, CME, retirement plans and malpractice coverage.

Qualifications

• MD or DO with Board Certification/Board Eligibility in Family Medicine.
• Active or eligible for Oregon medical licensure.
• Minimum of 2 years of post-training primary care experience; 5+ years encouraged.
• Strong foundation in comprehensive adult primary care and chronic disease management.
• Experience with routine outpatient procedures; women’s health valued.
• Interest in preventive medicine, population health and value-based care.
• Collaborative, flexible, team-oriented approach with evolving processes.

Practice Highlights

• Lower patient volume and thoughtful appointment lengths to deliver exceptional and individualized care
• Adult primary care population, ages 18+, with an emphasis on preventive care, chronic disease management, wellness, and population health
• Opportunity to provide a broad range of primary care procedures and women's health services
• Collaborative care environment with APP, behavioral health, clinical, operational, and population-health support
• Very light shared call

Compensation & Benefits

• Guaranteed base salary with a 2-year guarantee: $275,000 - $300,000 DOE
