Qualifications

• MD/DO licensed to practice in Illinois.
• Graduate of an accredited medical school.
• Current CPR/ACLS/PALS certifications.
• Board certification or eligibility in Family Medicine.

Responsibilities

• Provide family medicine care as part of a physician-led medical group.
• Work 0.8 FTE in a full-time role.
• Deliver care across an integrated health system using Epic EMR.

Qualifications

• Doctor of Medicine or Doctor of Osteopathic Medicine with current license to practice medicine in the state of Illinois
• Graduate of accredited and recognized medical school
• Current CPR certification or specialty specific advanced life support (ACLS, ATLS, PALS) certification obtained through American Heart Association.
