Job summary

Ascension’s Greenfield Clinic in Greenfield, WI, seeks an outpatient primary care physician to join an established team. The clinic supports full-service LabCorp and Premier Radiology services and is attached to a microhospital for convenient access to primary care and emergency services.

You will provide patient-centered care across all ages, coordinate transitions, and contribute to quality initiatives within a joined healthcare system.

Qualifications

• One or more of the following credentials: MD/DO credentialed from the Minnesota Board of Medical Practice obtained prior to hire date or job transfer date.
• MD/DO credentialed from the Wisconsin Medical Examining Board obtained prior to hire date or job transfer date.

Responsibilities

• Deliver comprehensive, patient-centered primary care, including high-quality diagnostic and treatment services.
• Coordinate seamless care transitions for patients, managing specialized consultations and inpatient admissions.
• Lead or actively participate in continuous quality improvement initiatives to enhance efficiency and cost-effectiveness of care delivery.

This job location is currently located in a Health Professional Shortage Areas (HPSAs) and/or Medically Underserved Areas/Populations (MUA/P) Shortage Designation Type.

How you’ll make an impact in this role

• Join an established team of 2 Internal Medicine physicians, 4 Family Medicine physicians, 1 Internal Medicine APC, and 1 Family Medicine APC
• Each clinician is paired 1:1 with a Medical Assistant, supported by 5 front-desk receptionists and a shared RN.
• Patient population ranges from newborns to the elderly, covering all ages.
• The clinic offers a full-service LabCorp-managed lab and a full-service radiology department managed by Premier Radiology.
• A key advantage is its attachment to a microhospital, providing patients convenient access to primary care and emergency services in one location.
• Deliver comprehensive, patient-centered primary care, including high-quality diagnostic and treatment services, to improve the health and well-being of the community.
• Coordinate seamless care transitions for patients, managing specialized consultations, inpatient admissions, and collaborating with case management for successful discharge planning.
• Lead or actively participate in continuous quality improvement initiatives to enhance the efficiency and cost-effectiveness of care delivery within the clinic and the wider system.
• Maintain complete and timely patient documentation and electronic medical records to ensure compliance, accurate records, and support the financial integrity of the practice.
• Ability to practice autonomously and demonstrate comfort managing a solo outpatient primary care panel within a clinic environment

What Minimum Requirements You’ll Need

Licensure / Certification / Registration

• Required Credential(s):
  • One or more of the following:
  • Physician MD/DO credentialed from the Minnesota Board of Medical Practice obtained prior to hire date or job transfer date.
  • Physician MD/DO credentialed from the Wisconsin Medical Examining Board obtained prior to hire date or job transfer date.
• Preferred Credential(s):
• Advanced Life Support. American Heart Association or American Red Cross accepted.

Education

• Doctor of Medicine (MD) or Doctor of Osteopathy (DO) required.
