Qualifications

• Graduation from an accredited medical school.
• Completion of an accredited United States Family Practice Residency Program.
• Board Certified or Board Eligible in Family Medicine.
• Valid Arizona medical license.
• Valid DEA certificate.
• Strong communication with patients and families.
• Ability to provide respectful, culturally responsive care.
• Basic business English, spelling, and punctuation.
• Valid Arizona driver’s license.
• CPR and AED certification for Healthcare Professionals.
