Job summary

True North Consultants, Inc. in New York is seeking a Family Medicine Physician to join a 100% outpatient practice across Long Island. The role offers flexible hours with Monday–Friday schedules, no on-call, and a highly competitive compensation in the $250k–$400k range.

You will provide comprehensive primary care, perform exams, order tests, manage treatment plans, document in EMR, and collaborate with a multidisciplinary team while meeting HIPAA and NYS DOH guidelines.

Qualifications

• Medical Degree (MD or DO) with active or eligible board certification in Family Medicine.
• Licensed (or eligible for licensure) in New York State.
• Strong diagnostic, communication, and interpersonal skills.
• Team-oriented with a patient-centered approach to care
• Proven ability to maintain and grow a local clinical practice is required
• Entrepreneurial mindset with an ability to grow and manage a thriving patient base.
• Familiarity with EMRs and value-based care metrics is a plus.

Responsibilities

• Serve as a key clinical provider in the delivery of high-quality care.
• Conduct comprehensive assessments including history taking and physical examinations.
• Order and interpret lab results and other diagnostic tests
• Create and oversee individualized treatment plans.
• Provide clear communication and education to patients regarding diagnoses, treatment options, and preventive strategies.
• Maintain thorough and accurate documentation within the Electronic Medical Record (EMR) system.
• Collaborate with referring physicians, specialists, and care teams to ensure seamless patient care.
• Provide clinical oversight to nurses, technologists, and support staff within the practice.
• Stay compliant with HIPAA, NYS DOH, CMS, and relevant payer guidelines.
• Participate in population health initiatives and support ongoing quality improvement projects.
• Contribute to the development of a sustainable patient panel aligned with practice growth objectives.
