Job summary

Top Echelon LLC is seeking a Family Medicine Physician for its Long Island, NY outpatient group known for clinical excellence and patient satisfaction. 100% outpatient with flexible hours, Monday–Friday, no call.

The physician will lead comprehensive exams, collaborate with a multidisciplinary team, and participate in quality improvement initiatives to grow a sustainable patient panel.

Qualifications

• MD/DO with active or eligible Board Certification in Family Medicine.
• Licensed to practice in New York State.
• Strong diagnostic, communication, and interpersonal skills.
• Team-oriented, patient-centered care approach.

Responsibilities

• Provide high-quality clinical care as a key provider.
• Perform history taking and physical examinations.
• Order and interpret labs and diagnostic tests.
• Develop and oversee individualized treatment plans.
• Educate patients on diagnoses, treatment options, and preventive care.
• Maintain thorough EMR documentation.
• Collaborate with referring physicians, specialists, and care teams.
• Provide clinical oversight to nurses and support staff.
• Ensure HIPAA, NYS DOH, CMS, and payer compliance.
• Participate in population health initiatives and quality improvement.
• Grow and sustain a patient panel aligned with practice growth.

Family Medicine Physician

Compensation: $250K-$400K per year

Job Type: Flexible hours, Monday–Friday, No call.

A well-established, physician-led group with locations throughout Long Island, is currently seeking to hire a Family Medicine Physician. This practice has built a stellar reputation for clinical excellence, patient satisfaction, and multidisciplinary collaboration. 100% outpatient.

Key Responsibilities:

• Serve as a key clinical provider in the delivery of high-quality care.
• Conduct comprehensive assessments including history taking and physical examinations.
• Order and interpret lab results and other diagnostic tests
• Create and oversee individualized treatment plans.
• Provide clear communication and education to patients regarding diagnoses, treatment options, and preventive strategies.
• Maintain thorough and accurate documentation within the Electronic Medical Record (EMR) system.
• Collaborate with referring physicians, specialists, and care teams to ensure seamless patient care.
• Provide clinical oversight to nurses, technologists, and support staff within the practice.
• Stay compliant with HIPAA, NYS DOH, CMS, and relevant payer guidelines.
• Participate in population health initiatives and support ongoing quality improvement projects.
• Contribute to the development of a sustainable patient panel aligned with practice growth objectives.

Qualifications:

• Medical Degree (MD or DO) with active or eligible board certification in Family Medicine.
• Licensed (or eligible for licensure) in New York State.
• Strong diagnostic, communication, and interpersonal skills.
• Team-oriented with a patient-centered approach to care
• Proven ability to maintain and grow a local clinical practice is required
• Entrepreneurial mindset with an ability to grow and manage a thriving patient base.
• Familiarity with EMRs and value-based care metrics is a plus.
