Job summary

ChesPenn Health Services is seeking a Board‑certified Family Nurse Practitioner to deliver comprehensive primary care to diverse patient populations across Chester and Delaware Counties. You will provide preventive services, manage chronic diseases, and coordinate care alongside a multi‑disciplinary team.

Qualifications

• Master’s or Doctorate in Nursing required.
• Active PA nurse practitioner license in Pennsylvania.
• Board certification as Family Nurse Practitioner (FNP).
• Experience in community health, outpatient care, or underserved populations preferred.
• Proficiency in EMR systems and strong documentation skills.
• BLS certification required; ACLS/PALS a plus.

Responsibilities

• Provide comprehensive primary care to patients of all ages, including preventive services and chronic disease management.
• Conduct thorough patient assessments and develop individualized treatment plans.
• Prescribe medications and coordinate care with other healthcare professionals.
• Educate patients and families on health promotion, disease prevention and lifestyle changes.
• Document patient encounters and outcomes using EMR systems.
• Participate in quality improvement initiatives and stay current with clinical best practices.
