Responsibilities

  • Provide routine and preventive care, including physical exams and screenings.
  • Manage acute and chronic conditions in an outpatient setting.
  • Conduct follow-up visits for patients referred to specialists.
  • Educate patients on health maintenance and ongoing care.
  • Collaborate with nurses, medical assistants, and administrative staff.
  • Supervise nurse practitioners and physician assistants.
  • Participate in emergency call coverage rotation, including weekends and holidays.
  • Maintain patient confidentiality and professional conduct at all times.
  • Perform additional duties as assigned by supervisor.

Position Summary

ChesPenn is seeking a dedicated and patient-centered Family Medicine Physician to provide comprehensive primary care to adults and children at our Chester, PA location. This role is ideal for a physician who thrives in a collaborative, community-focused environment and is passionate about serving medically underserved populations.

Qualifications
  • Graduate of an accredited medical school
  • Completion of an approved residency program in Family Medicine
  • Unrestricted Pennsylvania medical license
  • Board eligibility required; board certification required
  • DEA certification
  • Current COVID-19 and flu vaccinations required
  • Experience in ambulatory primary care or community health preferred

  • Familiarity with trauma-informed care principles preferred
  • Ability to work effectively as part of a multidisciplinary healthcare team

Scope of Responsibilities
Clinical:
  • Provide routine and preventive care, including physical exams and screenings

Productivity:
  • Maintain productivity levels in accordance with BPHC grant standards
  • Meet targets for patient encounters and visits per patient annually
Record-Keeping & Administration:
  • Document patient care accurately in the Electronic Medical Record (EMR) system
  • Build relationships with community agencies and hospitals
  • Lead or support medical and public health initiatives as needed
  • Contribute to the development of the annual healthcare plan
  • Engage in continuing medical education and professional development
