United Health Services (UHS) seeks a board-certified Nurse Practitioner or Physician Assistant for a full-time Family Medicine Provider Float. Base at UHS Primary Care Upper Front Street with travel within 30 miles to Greene and Clinton Street centers.

Monday–Friday schedule, licensure in NY required, taking over patient panels during provider absences. UHS offers NCQA recognition, a supportive culture, and ample opportunities for telemedicine from a home base.

Qualifications

• Board-certified NP or PA with NY licensure.
• FNP for NPs; PA must be licensed in NY.

Responsibilities

• Cover patient panels during provider absences (disability, FMLA, vacation).
• Perform acute telemedicine visits from home base.
• Balance patient care with family commitments while maintaining quality care.
