Job summary

Qualifications

• Completion of a graduate level nurse practitioner program accredited by ACEN or CCNE.
• ANCC or AANP certification.
• Licensed Registered Nurse (RN) by the Texas Board of Nursing (TBON).
• Licensed Advanced Practice Registered Nurse (APRN) in a TBON-sanctioned specialty.

Responsibilities

• Works independently to full scope of practice under Texas Nurse Practice Act with collaborating physician access.
• Performs comprehensive history and physical examinations for diverse populations.
• Orders and interprets diagnostic tests as appropriate to patient concerns.
• Diagnoses acute, chronic, and episodic illnesses and develops care plans.
• Implements evidence-based treatment plans including medications and non-pharmacological approaches.
• Educates patients about health diagnoses and preventive care tailored to their socioeconomic context.
• Monitors and adjusts care plans to ensure cost-effectiveness and outcomes, seeking feedback from patients and team.
• Facilitates transitions of care across services for seamless communication among teams.
• Documents care plans and patient encounters timely for continuity of care.
• Complies with licensure requirements, quality initiatives, and safety training.
• Contributes to population and individual health through prevention and chronic disease management.
• Performs related duties such as data collection, staff training, and compliance activities as needed.
