Job summary

You will engage in quality assurance activities, adhere to state nursing practice acts, and participate in continuous improvement to ensure optimal patient outcomes and safety.

Qualifications

• Completion of a graduate level Nurse Practitioner program accredited by ACEN or CCNE.
• Certification by ANCC or AANP.
• Licensed RN by TBON.
• Licensed APRN in a TBON-identified specialty.

Responsibilities

• Works independently to full scope of practice per Texas Nurse Practice Act with collaborating physician availability.
• Performs history and physicals for diverse populations, considering social determinants of health.
• Orders and interprets diagnostic tests.
• Diagnoses acute, episodic, and chronic diseases.
• Develops comprehensive care plans aligning with guidelines and regulations.
• Prescribes pharmacological and non-pharmacological treatments.
• Provides education for health conditions and preventive care.
• Monitors plan of care for cost-effectiveness and quality outcomes.
• Facilitates patient transitions between care settings and communicates with the health care team.
• Documents timely for review by interdisciplinary team.
• Supports population and individual health through prevention and chronic disease management.
• Performs data collection, staff meetings, training, and compliance activities.
