• Performs history and physical examinations for diverse populations
• Orders and interprets diagnostic testing
• Diagnoses acute, episodic, and chronic diseases
• Develops a comprehensive plan of care including guidelines and regulations
• Prescribes pharmacological and non-pharmacological treatments
• Provides education on health diagnoses and prevention
• Monitors plan of care for cost-effectiveness and outcomes
• Facilitates patient transitions between services
• Documents timely for review by interdisciplinary team
• Adheres to internal controls and licensure requirements
• Supports population health through prevention and chronic disease management
• Performs related duties including data collection, staff training and compliance activities
